Work accident – for ENS agents

You are a civil servant, a student or a contractual employee with a contract equal or superior to one year

1. The form ” declaration of accident of service or work ” 
to be completed and signed by your superior.

2. The complementary certificate of commuting accident (if it is a commuting accident)
compulsory and must be accompanied by a precise map of the area or place where the accident took place.

3.The accident at work certificate
allows you to consult a doctor as soon as possible (preferably on the day of the accident) without advancing the consultation fees.

It must be completed and signed by your supervisor before you go to your medical consultation.

This form must be presented to all health professionals (doctor, pharmacist, physiotherapist, hospitals, etc.) to avoid having to pay in advance. It must be kept until the end of the treatment (doctors can make a copy).

Imperative: you must not use your “carte vitale” or pay for any care.

If you are asked to advance your medical expenses, please keep a receipt and send it to your unit manager as soon as possible.

Do not send anything to the social security or to your mutual insurance company.

 

You are a contract employee with a contract of less than one year

1. The “work accident declaration” form
2.The work accident or occupational disease form

You must fill it in and keep it. It will be systematically presented to all the health professionals who will complete the table on page 2 as the care is provided.

3. Medical certificate transmission
During your medical consultation, the doctor will give you an initial medical certificate.
►You will send sections 1 and 2 to your social security
►You will keep section 3
►You will send section 4 to the HR department

Thereafter, your entire file will be managed by your social security center, which will refund medical consultations, care and all medical expenses.
