The form “declaration of accident of service or work”
►Within 15 days after the medical finding,
Fill in and sign the document in 2 copies, have it signed by your line manager then send it to the HR department.
The HR department will inform the SRH of Sorbonne University.
The certificate of coverage for work-related injury
► Complete and have your supervisor sign the document before you go to your medical consultation.
This document allows you to consult a doctor as soon as possible (preferably on the day of the accident) without advancing the consultation fee.
It will be imperative to present it to all health professionals (doctor, pharmacist, physiotherapist, hospitals…) to avoid advancing fees.
It must be kept until the end of the treatment (doctors can make a copy).
